Parents/Guardians,

If your student does not attend BMS and would like to attend the BMS Valentine’s Dance on Friday, February 13, a completed BMS School Dance Procedures Form is required. You can download a copy of this form by clicking this link: https://aptg.co/1d8tcc

The completed form and all required documentation must be turned in to the BMS Office by 12:00 PM on Wednesday, February 11th for your student to be eligible.

To qualify, students must:

Have participated in a BMS activity (such as Band) or represented BMS in a school-sponsored sport during the 2025–26 school year

Provide proof of current academic eligibility (no more than 2 D’s or 1 F)

Submit current emergency contact information

Students who do not meet all eligibility requirements or miss the deadline will not be permitted to attend.
